Chronic Disease Self Management Workshops (Stanford University model)

The Health Planning Council offers this award-winning tool to County residents.

This six-week workshop provides adults, ages 18 and above, who have an ongoing health condition (such as Arthritis, Cancer, Diabetes, Obesity, Parkinson's, any long-term health condition) with strategies for living a healthy lifestyle.

Topics that are covered include physical activity, exercise, healthy eating, dealing with emotions, managing medications and much more.

In this workshop participants will learn and try new skills. Skill categories include:
1. Skills needed to deal with specific condition
2. Skills needed to continue your normal life
3. Skills needed to deal with emotions

Diabetes Self-Management workshops The Health Planning Council offers this 6 week program for people with Type 2 Diabetes, Type 1 Diabetes or Pre-Diabetes. (Approved by the American Diabetes Association)

This Stanford University Program has been developed to help individuals who have Type 2 Diabetes, Type 1 Diabetes or Pre-Diabetes. It will help with skills needed to manage Diabetes as well as to deal with ways continue to live a healthy life.
